Below are links to forms leaders often need for trips.
Release Agreements

Volunteer Release Agreement. All trip participants need to read and sign a Volunteer Release Agreement for every trip. Leaders should print out several copies to bring with them and ask participants to read and sign the form before starting the trip. After the trip, return the release form to the AMC at the address listed on the form. For more information on why this form is necessary, please read this memo from the Risk Management Team.

Release Agreement for AMC’s Volunteer-Managed Facilities. This similar release applies to guests of AMC’s Volunteer-Managed Facilities.

Release Agreement for AMC Major Excursions. This release agreement is used for AMC Major Excursion trip participants.
Reporting & Communications Information

AMC Incident Report. Sometimes accidents or other unexpected events occur on an AMC-led trip. Should this happen, leaders should submit an Incident Report using this form. Learn more about when to use the Incident Report Form.

White Mountain National Forest Summary of Use Report. Anytime a trip takes place in the White Mountain National Forest, the leader should fill out this form and send it to the address listed.
