Requirements for Leaders of FortyPlus Activities
    This page provides requirements for initiating or cancelling an Activity.

AMC Waiver Form   This Form was updated April 29, 2006. Make sure you are using the updated version.
    Use of this form is a firm requirement for every Activity. It provides some degree of protection for both AMC and it's leaders (that's you) against legal action. Important! Upon completion of an activity, leaders must mail the original of this form to the Joy Street address indicated on the bottom of the form and keep a copy for themselves. They should indicate in their trip report that the form has been sent. The form is in Adobe PDF format. You need Adobe Reader to download the form.

Trips in the White Mountain National Forest(WMNF)
. Because there appears to be some confusion about White Mountain National Forest (WMNF) trip requirements 2 points need to be clarified:
       1) WMNF Guide Cards are required for all trips that take place within the WMNF, not just wilderness areas;
       2) all trips in the WMNF require that a trip report be sent to mleberman@fs.fed.us, as well as the activity group coordinator or other point person designated to receive trip reports.

       40+ receives WMNF Guide Cards each year from the Boston Chapter.  So, if you will be needing one, please let your Activity Coordinator know.  This applies most often for hiking but is also applicable to any other trip in the WMNF.  Failure to have a card can result in the card-less leader being personally assessed a fine by a ranger.  A link to the WMNF trip report form is below.  It includes M. Leberman's e-mail and postal address.
